 Sports PhysicalsStudents MUST have a current sports physical on file before they may attend team tryouts. The form may be printed from the link below. Middle School Athletic Participation Form.
All schools have a sports physical day early in September. You may go to your family doctor or go to any school to obtain your physical. IMPORTANT: PARENTS, BE SURE TO SIGN ALL SIGNATURE SPACES ON THE FORM including the last page!! Many students miss the first days of team tryouts due to missing signatures. Physicals are valid from May 1 through the following school year. |
